APPLICATION FOR MEMBERSHIP

Prospective members must be brought in by a current member. Call number at upper right or email George Holevas gholevas@ptd.net for details and sponsorship --if you need a sponsor.

REQUIRED:
1. A filled out and signed Application Form .doc format or .pdf format.

2. No duplicate or conflicting categories allowed although some members have agreed to split their category into separate subcategories. (for an example, see our insurance category on member page.) Current members are polled for conflicts prior to your visit.

3. Applicants who have completed the above will be asked to make a presentation for membership approval. Pledge Day is normally the first Wednesday of the month.

If your category is taken, you can ask to be placed on our waiting list.
